Payment Policy
Payment is requested at the time services are rendered. For your convenience,
we accept MasterCard, Visa, Diners Club, American Express, ATM cards,
personal checks, money orders or cash.
We participate in a variety of HMO and PPO insurance plans. In accordance
with the terms of your HMO/PPO plan, the patient co-payment is due at
each visit.
As a courtesy, we will bill your insurance at no charge. In order to provide
this service, we must have all of your current insurance information.
It is important to note that ultimately, you are responsible for your account.
